As said quite a few times about the place, all it means is the mail server didn't respond. Its nothing wrong with the code/site beyond the fact that the mail server is down when you click submit.

Any mention of smtp or mail can be ignored unless it happens over a period greater than a couple of hours.

You won't get these if you don't have mail notification selected in your profile, unless the receiptiant has the field checked.

If you don't use the mail function, its best to just turn it off anyways.

twas the mail server down @John, everyone in a thread has to have it disabled. The way phpbb works is it checks to see if anyone has the topic flagged, if they do the person who submits -> SMTP.php and a mail is sent out.

So unfortunately if the mail server ever goes down there's nothing much you can do.
