You need to use Nero Recode or similar to do this. There will be great quality loss, which will be visible especially if you have a HDTV / Projector / plasma.

The best option is a standalone player. About £40 and a bit of effort will get you a really great setup:

Depends. What do you mean with playable DVD?

If you already have a standalone DVD player that can play AVI (MPEG-4/XviD/DivX) files and you want all extras:

1.) Mux the AC3 commentary to the main movie AVI file
2.) Burn the AVI and the sub/idx file (all with the same name) to a DVD

Hopefully your standalone player can play the subtitles too. I quick search for my standalone brought up this:

If you want to create a normal video DVD just like the one from which these files are ripped I would suggest:

1.) Use TMPEGEnc to convert AVI back to MPEG-2.
2.) Use a DVD authoring program of your choice to create a DVD out of the MPEG-2 file (e.g. TMPEGEnc DVD Author).

Very underrated flick. The scene where the person is filming the demon/devil thing in the church doorway is one of the creepiest horror movie scenes.
